PCA Forms for Tour/Driving Events
Event Insurance form/submission (edition 2017) Note: this must be submitted for insurance “issuance” BEFORE an event, best at least one week in advance for any non-speed event. This is a submission form, that is, once filled out on-line, it is submitted directly to PCA. You must log onto your PCA account to access this form. The link will bring you to PCA.org if you are not logged on and you will be given a log in screen. Once logged on, your form will appear.
Release and Waiver of Liability Print out in color — need to show the RED lettering. Every person attending the event must sign this form before participating.
Minor and Parent Form Waiver Release form for Minors Needed for a minor to participate in any event.
Observer Report for Driving Tour (edition 2017) Note: This report should be completed no later than ten days after the event, and should not be completed by the tour leader. This is a submission form and you must log onto your PCA account to access this form.
Incident Report for Driving/non-Driving Events (edition 2017) Note: In the case of a multiple car incident, complete one report per car. Incidents involving bodily injury are to be submitted on next business day. Other incidents are to be reported within five (5) business days. This is a submission form and you must log onto your PCA account to access this form.
Post Event Report (edition 2017) This report is a simple report to PCA Safety Chair indicating whether there was any safety related “incidents” during an event. The report should be submitted within 5 days of the event. This is a submission form and you must log onto your PCA account to access.
